Scoped engagement / one operational breakdown

    Operational Breakdown Assessment

A scoped review of one operational problem: intake, evidence and workflow mapping, causal and ownership analysis, and a prioritized repair plan.

For recurring referral failures, access gaps, unowned handoffs, conflicting reporting definitions, lost operational knowledge or implementation that has stalled.

The initial discussion establishes fit. Scope, dependencies and fees are agreed before paid work begins; no outcome or turnaround is guaranteed.

    Intended buyer

For the person accountable for the work

Medicaid health plans, behavioral health organizations, housing / homelessness programs and human-service operators. Intended for an operations, program, quality or reporting lead who can involve the people responsible for the work.

    Scope & deliverables

    What the assessment covers

    01

    Intake and scope

Define one recurring breakdown, the decision or service it affects, what has been tried, and the boundaries of the review.

You receive: Agreed problem statement, scope, evidence request and stakeholder list.

    02

    Evidence and workflow mapping

Review agreed policies, redacted artifacts and reporting definitions. Trace the actual work from intake through handoffs, exceptions and closure with staff.

You receive: Current-state workflow and evidence map, including source gaps and dependencies.

    03

    Causal and ownership analysis

Test competing explanations against the available evidence. Separate policy, information, capacity and handoff problems; identify decisions without an accountable owner.

You receive: Supported findings, hypotheses still to test, ownership gaps and validation questions.

    04

    Prioritized repair plan

Sequence proposed repairs by operational consequence and dependencies. Identify who must approve, carry out and check each change.

You receive: Repair plan with proposed owners, dependencies, acceptance checks and a bounded next-step scope.

    Prepare for the discussion

Bring the problem, not sensitive records

    • One workflow or decision that repeatedly breaks down.
    • Its operational consequence and what has already been tried.
    • The people, policies, reports and systems involved.
    • Known constraints and who can authorize changes.

Do not send confidential client or member information, health records or identifiable case details through this site. Evidence access and data-protection requirements must be agreed separately.

    Boundaries

The assessment identifies and prioritizes repairs; it does not include an unlimited implementation, software replacement, clinical decisions, legal advice, compliance certification or a guarantee of provider availability.

Findings distinguish supported causes from unresolved hypotheses. Any implementation or prototype requires its own agreed scope and accountable owner.
