Rural access operations
Rural Medicaid access and care-coordination operations
In rural counties, a member can be eligible for a service and still unable to reach it. Iterum designs the coverage views, decision rules, and escalation paths that let care-coordination teams see where access breaks and act on it — in Nevada and other rural and frontier markets.
Built for
- Medicaid managed care and care-coordination teams
- Rural and frontier health programs
- Behavioral health and community service networks
Where it breaks down
Coverage looks adequate on paper
Provider lists count contracted sites, not whether a member can realistically reach one. Distance, drive time, and appointment availability get lost.
Pharmacy and specialty gaps surface late
Gaps usually appear when a coordinator is already on the phone with a member who has no nearby option.
Transport is a constraint, not a footnote
Long distances, limited non-emergency transport, and weather change which services are actually usable.
Escalation depends on who is working
Without shared decision rules, the same situation gets handled differently by different staff.
What we design
- 01
Service delivery coverage assessment
Map members, providers, and pharmacies by county so access is judged by reach, not by contract count.
- 02
Distance and transport constraints
Make travel burden visible next to each service so planning reflects field conditions.
- 03
Escalation decision support
Define when a coordinator escalates, to whom, and with what documentation — written so it holds across staff changes.
- 04
Network access gap review
Summarize where gaps cluster so network and program leads can prioritize recruitment, telehealth, or transport options.
Working examples
- Rural Tool — New Mexico Demo(opens in a new tab)
Public demonstration of rural complexity planning and decision support using New Mexico geography. Demonstration configuration, not a client deployment.
- Rural Tool system overview
How the demo is structured: complexity index, decision assist view, county analysis.
- Nevada rural behavioral health access platform
Historical Nevada interface from prior professional work, separate from the New Mexico demo and not an Iterum client engagement.
- ILOS Operations Framework
Turning Medicaid housing-benefit policy into one operating standard across partners.
